At Evergreen Mind & Medicine, we believe in transparent pricing and straightforward billing. This policy outlines how payments are handled for all appointments.

Payment Methods

We currently accept payment by cash, credit card, or debit card, through secure, HIPAA-compliant systems at the time of service.
Payments are processed through approved platforms only.

Cash-Pay (Self-Pay) Appointments

Evergreen Mind & Medicine is accepting cash-pay (self-pay) patients.
All services are billed directly to the patient at the time of the appointment.

Patients may request a Good Faith Estimate and a superbill for potential out-of-network reimbursement through their insurance provider.

Medical Cannabis Payment Policy

Medical cannabis evaluations and renewals are paid directly by the patient at the time of appointment.

Cancellation & No-Show Policy

Your appointment time is reserved specifically for you.

  • Appointments canceled with less than 24 hours’ notice or missed without notice will be charged a $50 Fee.

  • If you need to reschedule, please contact us at least 24 hours before your appointment time to avoid being billed.

Refunds

All payments are considered final once services are rendered.
Deposits for evaluations or consultations are non-refundable as they cover administrative time and provider preparation. Refunds may be issued only in cases of administrative error or duplicate payment.
