Who Qualifies for Medical Cannabis in Utah?

Patients may qualify for medical cannabis in Utah if they have one or more of the following conditions, as outlined by the Utah Department of Health & Human Services.

  • Chronic or acute pain (lasting longer than two weeks or not effectively managed by other treatments)

  • Persistent nausea not related to pregnancy, cannabis use, or a non-qualifying condition

  • Cancer (with symptoms such as pain, nausea, or weight loss)

  • HIV or AIDS

  • Alzheimer’s disease or dementia

  • Amyotrophic Lateral Sclerosis (ALS)

  • Cachexia or wasting syndrome

  • Crohn’s disease or ulcerative colitis

  • Epilepsy or debilitating seizures

  • Multiple sclerosis (MS) or muscle spasms

  • Post-traumatic stress disorder (PTSD) (as diagnosed by a licensed mental health provider)

  • Autism spectrum disorder

  • Terminal illness (with a life expectancy of less than six months)

  • Rare or incurable conditions that cause significant impairment of quality of life

  • Persistent pain due to neuropathy, fibromyalgia, or similar disorders

  • Other conditions approved by the Utah Compassionate Use Board (for patients under 21)

Patients under 21 years old require Compassionate Use Board approval and a legal guardian proxy.

Cannabis Renewal

30 minutes · $69

For patients with an active Utah medical cannabis card. Renewals include a brief check-in, symptom review, and certification update in the Utah Electronic Verification System (EVS).

If this is your first time meeting with Chandler Reed, PMHNP, your renewal appointment must be completed in person to establish care and verify identity per Utah DHHS guidelines.

After your initial visit, future renewals can be completed virtually anywhere in Utah for convenience.

Q: How long does it take to get approved?
A: Most patients receive approval within 1–2 business days.

Q: Can I renew my card online?
A: Yes. Once you’ve met in person for your initial evaluation, all renewals can be done virtually.
